First Visit

We applaud you for taking the first step in achieving optimal periodontal health! We look forward to welcoming you into our practice. We enjoy the opportunity to provide an excellent level of care to all of our dental family.

We will review your medical and dental history and perform a comprehensive periodontal evaluation. X-rays will be taken of your mouth so that we can provide you with the best treatment recommendations. We will provide you with a written treatment plan with an insurance estimate if applicable prior to performing any treatment. Any patient under the age of 18 must have a parent or guardian with them at the time of their consultation appointment.

•	Please bring your referral slip and any x-rays from your referring dentist.
•	Bring a list of your current medications—you can print out your patient registration and medical history ahead of time on our website. Please complete these forms and bring them with you to your first visit.
•	If you have a medical condition or concern( i.e. Artificial joints or heart valves that require antibiotic premedication, severe diabetes, uncontrolled blood pressure) please notify our office before your appointment.
•	Please notify us ahead of time if you have dental insurance that you would like to use. We will attempt to contact your insurance provider ahead of time to give you the most accurate information available to us.
